How To Acquire A COVID-19 Vaccine in Fairfax County
To receive a vaccine, you must sign up through the Department of Health. In Fairfax County follow this link.
Once there you have two steps:.
STEP 1: COMPLETE PRE-SCREEN AND VACCINE APPOINTMENT QUESTIONNAIRE
The Pre-Screen provides us with basic contact information and confirms your eligibility for a vaccine, and will only take moments to complete. Once your eligibility is confirmed, you will complete a Vaccine Appointment Questionnaire. The questionnaire will take about 10 minutes to complete and you will need to be prepared to share:
- Contact information
- Demographics (birthdate, etc.)
- Answers to some medical history questions. This ensures you do not have any medical issues that would give reason to not be vaccinated
- An active email address
STEP 2: APPOINTMENT SCHEDULING
Within a few days, the Health Department will call or email you to schedule an appointment.
