Personal Medical Information Forms:
The above Personal Medical Information (PMI) forms are
provided for your convenience and safety. The forms provide you a
convenient and effective way to keep all of your important medical
history in one place. Fill it out and bring a copy each time you
visit a new doctor or dentist. If you use more than one doctor
(primary care, cardiologist, dentist, etc.), you should bring
the current form to all of your doctor appointments so that the doctor
treating you that day has the most current and accurate information
about your health and medications. This form can be extremely
important in the event of an emergency to provide emergency medical
personnel information about you that can affect the type of treatment
they provide. For example, if you take certain medications they can
react with others, affect your pulse rate, make you
bleed more than normal, etc.
*Please bring any recent EKG's, blood work and office
notes from your family doctor with you to your first visit.
